7/22/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a content mood evident by her facial expressions and overall conversation with QMHPA. QMHPA engaged Kaila in a discussion surrounding how her day was going. Kaila expressed to QMHPA that her day was going well. Upon arrival, she did not present any symptoms of S/I. Intervention: QMHPA began the session by inquiring about Kaila’s current thoughts, feelings and emotions. QMHPA discussed with Kaila about situations that may cause her to become frustrated, overwhelmed, or depressed. QMHPA encouraged Kaila practice appropriate coping strategies to overcome anxiety, frustration or when there is external stimulation. QMHPA conducted a skills building activity. QMHPA informed Kaila that the purpose of this activity was to guide Kaila through breathing exercises, meditation, and stretching techniques. QMHPA questioned Kaila about identifying triggers that cause her to become frustrated. QMHPA demonstrated with Kaila appropriate responses to triggers through role play. QMHPA reviewed with Kaila enjoyable activities she would like to endure in to overcome anxiety. Response: Kaila appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Kaila expressed to QMHPA that her daughter and her son pluck her nerves at times. Kaila said she be worried about her daughter at times walking around the neighborhood with all the violence. Kaila said she sticks with reading the Bible and listening to the word keeps her calm. Kaila expressed to QMHPA that she doesn’t understand why she continues to put herself in the situations that she does to cause her to become depressed or experience anxiety but she does.
